Everytime we go to the blood bank to donate blood, we have to list the countries we have visited over the past 3 years. A lot of us travel often and it is very difficult to remember where we have travelled, unless we bring our passport along. Also, the form doesn't provide enough space to list the countries so I had to end up scribbling all over the place.

If we had made a complete declaration in a previous donation and was allowed to donate, do we still need to declare again if we return after 3 months? Can we just state "same as previous donation on [date]"? If the declaration is the same and we could donate then, what is to stop us from donating this time, if we didn't visit any other country since the last donation?

I am heading down to the blood bank this evening but I forgot my passport, again. The last time, I had to call home and ask my husband to read out all the countries in the passport. :?

You only need to inform HSA doctor on your recent travel history since the last successful donation.

Please do not hesitate to clarify with your doctor on duty should you need any further clarification.
